    My Medical Record Privacy Notice

    My Medical Record (MyMR) is a personal health record (PHR) for patients. It’s used to support the provision of healthcare for several clinical conditions by a number of NHS organisations. MyMR stores and displays your health record for you and the medical professionals providing your healthcare.

    The purpose of the Privacy Notice is to inform you about the collection and use of your personal data by MyMR in accordance with the relevant legislation, in particular the General Data Protection Regulation (GDPR) which takes effect from 25 May 2018.

    • MyMR is owned and operated by University Hospital Southampton NHS Foundation Trust (referred to as the Data Controller and registered with the Information Commissioners Office (ICO), reference number Z4969884)

    • If you’re a patient or healthcare service user with another NHS organisation they are the Data Controller and University Hospital Southampton NHS Foundation Trust are the Data Processor for MyMR.

    • The Privacy Notice (link below) covers:

    1. WHO provides and controls the service and who your data is shared with.

    2. WHAT information is collected, the lawful basis for this and your rights.

    3. WHY your details are collected and processed.

    4. WHERE your information comes from.

    5. HOW your data is used, processed and retained (stored). How to contact us or lodge a complaint with us or with a supervisory authority.
